Seminar Registration Form

Thanks for your interest in SVTI seminars. Please use this page to register on-line. If for some reason you do not like to register 0n-line you can click on this registration form, print,  fill and fax it to SVTI at 408-573-0200. A registration confirmation will be emailed to you. Alternatively send email to information@svtii.com or call 408-573-0100 Onsite training and group discounts are also available. Call SVTI for details. IEEE members with valid IEEE membership receive $30 discount on seminars and $40 discount for 12 week courses.

         4. Click the "Submit Form" below. You will receive a confirmation email from SVTI.

 

Payment is due before classes start.  You can also send pay in check or cash. Seminar tuition is not refundable unless SVTI receives written notice of cancellation at least one week prior to the start date of the seminar and is subject to $50  service fee. Please make sure you are registered before making travel arrangements. SVTI reserves the right to cancel and/or postpone the seminar if the number of registered students is below a minimum. In this case the tuition will be fully refunded or if preferred by student will be used toward the future seminars. SVTI will not be liable for students travel expenses and any other expenses resulted from the seminar cancellation.
